I know the bibliography usually comes at the end, but some of you have asked for this so I’m posting it now.  Hope it’s helpful.  There are weblinks where the material is/was available online for free or as part of a journal subscription, or via Amazon for books.

p.s. If you don’t have a journal subscription it’s worth googling for the title of the paper.  Put the title in speech marks and add filetype:pdf at the end of the query.  If it’s on the net for free  anywhere at all, you should find it this way.
